Stat. tit. 21, § 21-142A-5","heading":"Restitution form","body":"The district attorney's office shall provide all victims,\n\nregardless of whether the crime victim makes a specific request,\n\nwith an official request for restitution form to be completed and\n\nsigned by the crime victim, and to include all invoices, bills,\n\nreceipts, and other evidence of injury, loss of earnings and out-of-\n\npocket loss. The crime victim shall provide all documentation and\n\nevidence of compensation or reimbursement from insurance companies\n\nor agencies of this state, any other state, or the federal\n\ngovernment received as a direct result of the crime for injury, loss\n\nof earnings or out-of-pocket loss.
