Any warden, superintendent or district supervisor within the\n\nDepartment of Corrections may make application to a judge for an\n\narrest warrant upon any prisoner escaping from custody or\n\nconfinement in an institution or facility of the Department of\n\nCorrections or from house arrest or the Preparole Conditional\n\nSupervision Program. Said application shall be a statement verified\n\nby oath or affirmation alleging the occurrence of an escape.\n\nB. If the judge is satisfied that an escape has occurred, the\n\njudge shall affix his signature to a warrant of arrest of the\n\nprisoner.\n\nC. The person making application for the arrest warrant shall\n\ncause to be delivered as soon as possible, a copy of the issued\n\nwarrant of arrest of the prisoner to the court clerk, the district\n\nattorney and the sheriff's office within the geographical area where\n\nthe escape occurred.\n\nD.
