When judgment upon a conviction is rendered, the clerk must\n\nenter the same upon the minutes, stating briefly the offense for\n\nwhich the conviction has been had, and must immediately annex\n\ntogether and file the following papers, which constitute a record of\n\nthe action:\n\n1. The indictment and a copy of the minutes of the plea or\n\ndemurrer;\n\n2. A copy of the minutes of the trial;\n\n3. The charges given or refused, and the endorsements, if any,\n\nthereon; and\n\n4. A copy of the judgment, which shall include a notation of\n\nthe month and year of birth date of the defendant and the last four\n\ndigits of the Social Security number of the defendant. The judgment\n\nshall also contain the statutory reference, including any relevant\n\nsubsections, to the felony crime the defendant was convicted of, the\n\nclassification level of the felony crime the defendant was convicted\n\nof, the required amount of the minimum time to be served according\n\nto the sentencing court, and the date of the offense.\n\nB. The court shall obtain the month and year of birth date of\n\nthe defendant and the last four digits of the Social Security number\n\nof the defendant.\n\nR.L. 1910, § 5960.
