Applications for a credit service organization license shall\n\nbe under oath and shall state the full name and place of residence\n\nof the applicant. If the applicant is a partnership, the full name\n\nand place of residence of each member thereof shall be stated. If\n\nthe applicant is a corporation, the full name and place of residence\n\nof each officer or major stockholder thereof shall be stated. The\n\napplication shall give the approximate location from which the\n\nbusiness is to be conducted, and shall contain such relevant\n\ninformation as the Administrator may require.\n\nB. Each licensee shall maintain on file with the Administrator\n\na written appointment of a resident of this state as his agent for\n\nservice of all judicial or other process or legal notice, unless the\n\nlicensee has appointed an agent pursuant to another statute of this\n\nstate.
