If the consumer wishes to allow his or her consumer report\n\nto be accessed for a specific period of time while a freeze is in\n\nplace, he or she shall contact the consumer reporting agency using a\n\nmethod of contact designated by the consumer reporting agency,\n\nrequesting that the freeze be temporarily lifted, and providing, to\n\ncomplete the request, all of the following:\n\n1. Proper identification;\n\n2. The unique personal identification number or password\n\nprovided by the consumer reporting agency pursuant to subsection C\n\nof Section 3 of this act;\n\n3. The proper information regarding the time period for which\n\nthe report shall be available to users of the credit report; and\n\n4. The required fee, if applicable.\n\nB. A consumer reporting agency that receives a request from a\n\nconsumer to temporarily lift a freeze on his or her consumer report,\n\naccompanied by all of the items listed in subsection A of this\n\nsection, shall comply with the request no later than three (3)\n\nbusiness days after receiving the completed request.\n\nC.
