Stat. tit. 24, § 24-158","heading":"Notice of rights","body":"At any time a consumer is required to receive a summary of\n\nrights required under Section 1681g of Title 15 of the United States\n\nCode, the following notice shall be included:\n\n“Oklahoma Consumers Have the Right to Obtain a Security Freeze.\n\nYou have a right to place a “security freeze” on your credit\n\nreport, which will prohibit a consumer reporting agency from\n\nreleasing information in your credit report without your express\n\nauthorization. A security freeze must be requested in writing by\n\ncertified mail. The security freeze is designed to prevent credit,\n\nloans, and services from being approved in your name without your\n\nconsent. However, you should be aware that using a security freeze\n\nto take control over who gets access to the personal and financial\n\ninformation in your credit report may delay, interfere with, or\n\nprohibit the timely approval of any subsequent request or\n\napplication you make regarding a new loan, credit, mortgage,\n\ngovernment services or payments, rental housing, employment,\n\ninvestment, license, cellular phone, utilities, digital signature,\n\nInternet credit card transaction, or other services, including an\n\nextension of credit at point of sale. When you place a security\n\nfreeze on your credit report, you will be provided a personal\n\nidentification number or password to use if you choose to remove the\n\nfreeze on your credit report or authorize the release of your credit\n\nreport for a period of time after the freeze is in place. To\n\nprovide that authorization you must contact the consumer reporting\n\nagency by one of the methods that it requires, and provide all of\n\nthe following:\n\n1. The personal identification number or password;\n\n2. Proper identification to verify your identity;\n\n3. The proper information regarding the period of time for\n\nwhich the report shall be available; and\n\n4. The payment of the appropriate fee.\n\nA consumer reporting agency must authorize the release of your\n\ncredit report no later than three (3) business days after receiving\n\nall of the above items by any method that the consumer reporting\n\nagency allows.\n\nA security freeze does not apply to a person or entity, or its\n\naffiliates, or collection agencies acting on behalf of the person or\n\nentity, with which you have an existing account that requests\n\ninformation in your credit report for the purposes of reviewing or\n\ncollecting the account. Reviewing the account includes activities\n\nrelated to account maintenance, monitoring, credit line increases,\n\nand account upgrades and enhancements.\n\nYou have a right to bring civil action against anyone, including\n\na consumer reporting agency who willfully or negligently fails to\n\ncomply with any requirement of the Oklahoma Consumer Report Security\n\nFreeze Act.\n\nA consumer reporting agency has the right to charge you up to\n\nTen Dollars ($10.00) to place a freeze on your credit report, up to\n\nTen Dollars ($10.00) to temporarily lift a freeze on your credit\n\nreport, and up to Ten Dollars ($10.00) to remove a freeze from your\n\ncredit report.
