An envelope bearing an affidavit stating that the voter is\n\nqualified to vote, that he has personally marked the ballots or has\n\ndirected a person chosen by the voter to mark the ballots in\n\naccordance with the provisions of Section 7-123.3 of this title; and\n\n3.
