A registered voter who becomes physically incapacitated\n\nafter the deadline to request an absentee ballot as provided in\n\nSection 14-103 of this title and is unable to vote in person at the\n\nappropriate precinct on the day of the election may make a written\n\nrequest for an absentee ballot. Requirements for matching of name,\n\nbirth date, and identification number established pursuant to\n\nSection 14-105 of this title shall apply to a request for absentee\n\nballots under this section.\n\nB. 1. The request shall be signed by the voter or signed by a\n\nwitness at the voter’s direction if the voter is unable to sign his\n\nor her name, and shall be transmitted to the secretary of the county\n\nelection board.\n\n2. The person transmitting the request on behalf of the voter\n\nmay be anyone of the voter’s choosing who is at least sixteen (16)\n\nyears of age; provided, the person is not employed by nor related\n\nwithin the third degree of consanguinity or affinity to any person\n\nwhose name appears on the ballot.\n\n3. The person becomes the voter’s agent for purposes of voting\n\nby absentee ballot. The voter’s request must be accompanied by a\n\nsworn statement by a duly licensed physician. Expected or likely\n\nconfinement for childbirth on election day is sufficient cause to\n\nentitle a voter to vote absentee pursuant to this section. The\n\nphysician’s statement must attest to the fact that the voter is in\n\nfact unable to vote in person at the appropriate precinct on the day\n\nof the election because of a physical incapacity and that the\n\nphysical incapacity originated after the deadline to request an\n\nabsentee ballot as provided in Section 14-103 of this title.\n\n4. Upon receipt of the voter’s request and accompanying sworn\n\nstatement, the secretary of the county election board shall issue to\n\nthe voter’s agent the appropriate ballots and envelopes required for\n\nvoting by incapacitated voters.\n\n5. The ballots must be returned by the agent to the secretary\n\nof the county election board no later than 7:00 p.m. on the day of\n\nthe election. No person may be the agent for more than one voter at\n\nany election.
