The Secretary of the State Election Board shall devise and\n\ndistribute a registration application form to be used for\n\nregistering voters. Such registration application shall contain the\n\nfollowing information:\n\n1. The applicant's full name and date of birth, county and\n\nplace of residence, and mailing address pursuant to the provisions\n\nof subsection G of this section;\n\n2. A space or section to designate a political party recognized\n\nby the laws of the State of Oklahoma with which the applicant\n\nchooses to be affiliated;\n\n3. The Oklahoma driver license or identification card number if\n\nthe applicant has been issued a current and valid driver license or\n\nidentification card by Service Oklahoma, or if the applicant does\n\nnot have a valid Oklahoma driver license or identification card, the\n\nlast four digits of the voter's Social Security number;\n\n4. An oath of the eligibility of the applicant to become a\n\nregistered voter; and\n\n5. Such other information as may be deemed necessary by the\n\nSecretary to identify such applicant and to ascertain his or her\n\neligibility.\n\nB. A voter registration application shall be signed by the\n\napplicant in writing. The applicant shall personally subscribe his\n\nor her name to or make his or her mark on the application, and no\n\nagent, representative or employee of the applicant may sign or mark\n\non the applicant's behalf. The signature or mark must be the\n\noriginal, handwritten signature, autograph or mark of the applicant.\n\nNo facsimile, reproduction, typewritten or other substitute\n\nsignature, autograph or mark will be valid. Notwithstanding any law\n\nto the contrary, the Secretary of the State Election Board shall\n\nprescribe procedures to authorize any person incapable of personally\n\nmaking a mark to complete a voter registration application with\n\nassistance of an official of any voter registration agency or\n\nlicensed operator specified in Sections 4-109.2 and 4-109.3 of this\n\ntitle. Provided, for applications submitted electronically, in lieu\n\nof the signature requirements set forth in this subsection, the\n\napplicant shall consent to the use of his or her driver license or\n\nidentification card signature as provided in Section 4-109.4 of this\n\ntitle.\n\nC. Persons who do not indicate a recognized political party or\n\npolitical organization on their registration application shall be\n\ndesignated as Independents.\n\nD. Any person may apply in writing to the Secretary of the\n\nState Election Board for permission to print, copy or otherwise\n\nprepare and distribute the registration applications designed by the\n\nSecretary of the State Election Board. The Secretary may revoke any\n\nsuch permission at any time.\n\nE. All registration applications shall be distributed to the\n\npublic at no charge.\n\nF. The Secretary also shall prescribe procedures to accept and\n\nuse the National Mail Voter Registration Form, or its successor, as\n\nrequired by the National Voter Registration Act of 1993. Provided,\n\nto be accepted as a valid voter registration application, the form\n\nshall include the applicant's original, handwritten signature,\n\nautograph or mark as described in subsection B of this section.\n\nG. Applicants for voter registration or for change of voter\n\nregistration in any way shall provide a residence address and, if\n\ndifferent from the residence address, a mailing address. A\n\nresidence address shall include the street address of the residence,\n\nincluding a full house number, street name or number, apartment or\n\nsuite number, if applicable, and zip code. If a street address is\n\nnot available for the residence, applicants shall provide such\n\ninformation as the Secretary of the State Election Board deems\n\nnecessary for voter registration purposes. A post office box may\n\nnot be given as a residence address. A mailing address, which shall\n\ninclude the city and zip code, may be the actual emergency\n\nnotification or 911 address on file in the local community, a rural\ndress is\n\nnot available for the residence, applicants shall provide such\n\ninformation as the Secretary of the State Election Board deems\n\nnecessary for voter registration purposes. A post office box may\n\nnot be given as a residence address. A mailing address, which shall\n\ninclude the city and zip code, may be the actual emergency\n\nnotification or 911 address on file in the local community, a rural\n\nroute and box number, a post office box number or a street address.\n\nH.
