Forms to be used for filing Declarations of Candidacy shall\n\nbe prescribed by the Secretary of the State Election Board and shall\n\ncontain the following information:\n\n1. The name of the candidate as it shall appear on the ballot;\n\n2. The legal name of the candidate;\n\n3. The candidate’s place of residence and mailing address,\n\nwhich shall be kept confidential as provided in subsection B of this\n\nsection;\n\n4. The name of the office sought;\n\n5. The candidate’s date of birth;\n\n6. The party affiliation of the candidate seeking political\n\nparty nomination;\n\n7. The precinct and county wherein the candidate is a\n\nregistered voter;\n\n8. A voter registration verification form as provided in\n\nsubsection C of this section;\n\n9. An oath wherein the candidate swears or affirms that he or\n\nshe is qualified to become a candidate for the office sought, and\n\nthat, if elected, the candidate will be qualified to hold the\n\noffice; and\n\n10. Any additional information which the Secretary deems\n\nnecessary.\n\nB. The address of residence and the mailing address of a\n\ncandidate provided on a Declaration of Candidacy shall be verified\n\nby the secretary of the election board with whom the Declaration was\n\nfiled and shall be kept confidential; provided, however, such\n\naddress information shall be provided to a candidate, candidate\n\nrepresentative, or other lawful authority in anticipation of or as\n\npart of a contest of candidacy or contest of an election as provided\n\nfor in this title or as part of a petition challenge as provided by\n\nlaw.\n\nC. 1. Prior to filing a Declaration of Candidacy, a candidate\n\nshall obtain a voter registration verification form from the\n\nsecretary of the election board with whom the candidate is required\n\nto file. The candidate shall provide the form with the Declaration\n\nof Candidacy.\n\n2. A voter registration verification form shall include:\n\na. the candidate’s name, date of birth, address of\n\nresidence, party affiliation, voter identification\n\nnumber, the precinct and county where the candidate is\n\nregistered, and the original date of voter\n\nregistration within the county, if available, and\n\nb. the districts in which the candidate is registered to\n\nvote including but not limited to state senator and\n\nrepresentative, county commissioner, school and\n\ntechnology center, and municipality. If applicable, a\n\nward or member district shall be included for the\n\nschool or technology center district or municipality.\n\n3. The secretary of the election board with whom the candidate\n\nis required to file shall only accept a Declaration of Candidacy if\n\nthe information contained on the voter registration verification\n\nform matches the information provided on the Declaration of\n\nCandidacy, and if such Declaration meets all other requirements of\n\nstate law.\n\n4. The provisions of this subsection shall not apply to\n\ncandidates for federal office.\n\n5. The Secretary of the State Election Board shall prescribe\n\nthe voter registration verification form required by this\n\nsubsection.\n\nD. If the candidate has ever been ultimately determined by a\n\ncourt of proper authority to be guilty of an offense specified in\n\nsubsection A of Section 5-105a of this title or at the time of\n\nfiling the Declaration of Candidacy is named in an outstanding\n\nwarrant for arrest for such an offense, in this or any other state,\n\nthe candidate shall provide the following information on a form\n\nprescribed by the Secretary of the State Election Board:\n\n1. The name of the offense;\n\n2. The date of conviction or issuance of the outstanding\n\nwarrant; and\n\n3. The county and state of conviction or issuance of the\n\noutstanding warrant.\n\nThe provisions of this subsection shall not apply to an offense\n\nfor which the candidate has received a pardon.\n\nE. A Declaration of Candidacy form must be signed by the\n\ncandidate, and the signature must be properly notarized by a notary\noffense;\n\n2. The date of conviction or issuance of the outstanding\n\nwarrant; and\n\n3. The county and state of conviction or issuance of the\n\noutstanding warrant.\n\nThe provisions of this subsection shall not apply to an offense\n\nfor which the candidate has received a pardon.\n\nE. A Declaration of Candidacy form must be signed by the\n\ncandidate, and the signature must be properly notarized by a notary\n\npublic or other person authorized by law to administer oaths.\n\nF. In addition to the information required by this section, a\n\ncandidate may include a telephone number, email address and website\n\naddress.
