Each person appearing to vote shall announce that person's\n\nname to the judge of the precinct and shall provide proof of\n\nidentity, whereupon the judge shall determine whether the person's\n\nname is in the precinct registry. As used in this section, \"proof\n\nof identity\" shall mean a document that satisfies all of the\n\nfollowing:\n\n1. The document shows the name of the person to whom the\n\ndocument was issued, and the name substantially conforms to the name\n\nin the precinct registry;\n\n2. The document shows a photograph of the person to whom the\n\ndocument was issued;\n\n3. The document includes an expiration date, which is after the\n\ndate of the election in which the person is appearing to vote. The\n\nprovisions of this paragraph shall not apply to:\n\na. an identification card that is valid indefinitely, or\n\nb. an identification card issued by a branch of the armed\n\nservices of the United States to a person who is a\n\nmember of such branch or is retired therefrom; and\n\n4. The document was issued by the United States, the State of\n\nOklahoma or the government of a federally recognized Indian tribe or\n\nnation.\n\nProvided, if the person presents a voter identification card\n\nissued by the appropriate county election board, such card may serve\n\nas proof of identity without meeting the requirements of paragraphs\n\n2 and 3 of this subsection.\n\nB. 1. If a person declines to or is unable to produce proof of\n\nidentity, the person may sign a statement under oath, in a form\n\napproved by the Secretary of the State Election Board, swearing or\n\naffirming that the person is the person identified on the precinct\n\nregistry, and shall be allowed to cast a provisional ballot in a\n\nmanner consistent with the provisions of Section 7-116.1 of this\n\ntitle.\n\n2. A provisional ballot cast by a voter who declines or is\n\nunable to produce proof of identity shall only be considered\n\nverified and approved for counting if:\n\na. the voter's name on the affidavit substantially\n\nconforms to the voter's name in the voter registration\n\ndatabase, except as provided in paragraph 3 of this\n\nsubsection,\n\nb. the voter's residence address on the affidavit\n\nsubstantially conforms to the voter's residence\n\naddress in the voter registration database, except as\n\nprovided in paragraph 4 of this subsection,\n\nc. the voter's date of birth matches the information in\n\nthe voter registration database,\n\nd. the voter's Oklahoma driver license number or the last\n\nfour digits of the voter's Social Security number on\n\nthe affidavit matches the information in the voter\n\nregistration database. The provisions of this\n\nsubparagraph shall not apply if the voter was not\n\nrequired to provide a driver license number or the\n\nlast four digits of the voter's Social Security number\n\nat the time of registration, and\n\ne. the provisional ballot meets the eligibility\n\nrequirements set forth in Section 7-116.1 of this\n\ntitle.\n\n3. A voter casting a provisional ballot as provided in this\n\nsection who has legally changed his or her name, but has not updated\n\nthe name on the voter registry, may note this fact on the affidavit\n\nand submit a form prescribed by the Secretary of the State Election\n\nBoard to update his or her name. In such a case, and where the\n\nrequirements of Section 4-117 of this title are satisfied, the\n\nprovisional ballot shall be deemed to meet the requirements of\n\nsubparagraph a of paragraph 2 of this subsection.\n\n4. A voter casting a provisional ballot as provided in this\n\nsection who has changed his or her address of residence, but has not\n\nupdated the address on the voter registry, may note this fact on the\n\naffidavit and submit a form to update the address prescribed by the\n\nSecretary of the State Election Board. In such a case, and where\n\nthe requirements of Section 4-117 of this title are satisfied, the\n\nprovisional ballot shall be deemed to meet the requirements of\non who has changed his or her address of residence, but has not\n\nupdated the address on the voter registry, may note this fact on the\n\naffidavit and submit a form to update the address prescribed by the\n\nSecretary of the State Election Board. In such a case, and where\n\nthe requirements of Section 4-117 of this title are satisfied, the\n\nprovisional ballot shall be deemed to meet the requirements of\n\nsubparagraph a of paragraph 2 of this subsection.\n\n5.
