Stat. tit. 27A, § 27A-2-4-305","heading":"Suspension, revocation, or refusal to renew","body":"laboratory accreditation.\n\nA. The Department of Environmental Quality may suspend, revoke,\n\nor refuse to renew in part or in whole the accreditation of any\n\nlaboratory which does not continue to comply with Board of\n\nEnvironmental Quality rules or conditions of accreditation, or for\n\ncause, including but not limited to:\n\n1. The knowing and willful falsification of data submitted to\n\nthe Department;\n\n2. The misrepresentation or omission of material data in any\n\nreport submitted to any person relying on such report because of the\n\nlaboratory's accreditation;\n\n3. Failure to maintain or utilize approved quality control\n\nprocedures, recordkeeping, reporting procedures, methodology,\n\npersonnel requirements, equipment, facilities or analytical\n\ntechniques on which the accreditation was issued;\n\n4. Failure to achieve acceptable results on proficiency testing\n\nsamples; or\n\n5. For laboratories holding Department-issued accreditation,\n\nthe expiration, suspension or revocation of the laboratory's\n\nreciprocal out-of-state certification or accreditation.\n\nB.
