After the filing of the petition, the court may, on its own\n\nmotion or at the request of any party to the proceeding, if the\n\ncourt determines it to be for the best interest of the ward, order\n\nan evaluation of the subject of the proceeding in connection with\n\nany proceeding pursuant to the provisions of the Oklahoma\n\nGuardianship and Conservatorship Act where the capacity of said\n\nperson is a material issue.\n\nB. Any evaluations made pursuant to the Oklahoma Guardianship\n\nand Conservatorship Act, as appropriate for the condition or alleged\n\ncondition of the person being evaluated, shall be performed by:\n\n1. A physician;\n\n2. A psychologist;\n\n3. A social worker with a graduate degree in social work and\n\nfield training or experience in working with incapacitated or\n\npartially incapacitated persons; or\n\n4. Other expert with knowledge of the particular incapacity or\n\ndisability which the individual is alleged or has been found to\n\nhave, or knowledge of the skills required to meet the essential\n\nrequirements for the individual's physical health or safety or to\n\nmanage that individual's financial resources.\n\nC. An evaluation report prepared and signed by the person or\n\npersons performing the evaluation shall be submitted to the court\n\nprior to the hearing at which the court shall consider the report.\n\nThe report shall include, but not be limited to:\n\n1. A description of the nature and extent of the incapacity of\n\nthe person, if any;\n\n2. A description of the mental, emotional and physical\n\ncondition of the person, his ability to function in the ordinary\n\nactivities of daily life and, if appropriate, the educational\n\ncondition, adaptive behavior and social skills of the person;\n\n3. An opinion regarding the kind and extent of assistance, if\n\nany, required by the person;\n\n4. An assessment and review of any services necessary to\n\nprovide for the well-being of the person in the following areas:\n\na. physical health,\n\nb. mental health,\n\nc. social skills, and\n\nd. adequate and appropriate living conditions;\n\n5. An opinion regarding:\n\na. the probability that the extent of the incapacity, if\n\nany, of the person may significantly lessen or\n\nincrease, and\n\nb. the type of services or treatment, if any, appropriate\n\nfor the subject of the proceeding or which could\n\nfacilitate improvement in the condition of the subject\n\nof the proceeding; and\n\n6.
