Stat. tit. 30, § 30-3-122","heading":"Proposed plan for management of financial resources of","body":"ward.\n\nA. If not filed with the petition or submitted to the court at\n\nthe time of the hearing, within two (2) months after his\n\nappointment, a guardian or limited guardian of the property of an\n\nincapacitated or partially incapacitated person shall file with the\n\ncourt for its approval a proposed plan for the management of the\n\nfinancial resources of the ward that are under his management or\n\nadministration, and an inventory as required pursuant to Section 4-\n\n301 of this title. Said guardian or limited guardian shall submit\n\nsubsequent or modified plans as required by this title.\n\nB. Initial and subsequent guardianship plans for the management\n\nof the financial resources of the ward shall state:\n\n1. The services which are necessary to manage the property of\n\nthe ward placed under the control of the guardian or limited\n\nguardian;\n\n2. The means for obtaining those services;\n\n3. The manner in which the guardian or limited guardian of the\n\nproperty of the ward, the ward, and the guardian or limited guardian\n\nof the person, or if another individual has been appointed to serve\n\nin that capacity, will exercise and share decision-making authority;\n\n4. Such other services necessary to assist in the management of\n\nthe property placed under the guardian or limited guardian in\n\nfulfilling the needs of the ward and the duties of such guardian or\n\nlimited guardian, and the terms of the most recent dispositional\n\norder.\n\nC. Each such plan shall be substantially in the following form:\n\nPlan for the Management\n\nof the Property of the Ward\n\nI, ______________________, the (petitioner, guardian or limited\n\n(Name)\n\nguardian) for___________________________________________________\n\n(Name and current place of abode)\n\nhereby submit this (initial, annual or as ordered by the court)\n\nGuardianship Plan.\n\n1. I believe the services necessary to manage the property of the\n\nward which is subject to this Plan are as follows: ________________\n\n_______________________________________________________________\n\n2. Those services will be provided in the following manner:\n\n____________________________________________________________\n\n____________________________________________________________\n\n3. The guardian (or limited guardian) of the person, (Name, or\n\nindicate as not applicable) the ward, and I plan to cooperate and\n\nshare decision-making authority with regard to the ward within the\n\nprovisions of the dispositional order as follows:\n\n_______________________________________________________________\n\n_______________________________________________________________\n\n4.
