Stat. tit. 30, § 30-4-306","heading":"Report on guardianship or limited guardianship of","body":"property - Requirements - Attachments - Review of financial\n\nresources.\n\nA. A report on the guardianship or limited guardianship of the\n\nproperty of a ward shall set forth:\n\n1. The name and place of abode of the ward, and the name and\n\naddress of the guardian or limited guardian;\n\n2. If the ward is an incapacitated or partially incapacitated\n\nperson, significant changes in the capacity of the ward to manage\n\nhis financial resources and the services being provided to the ward\n\nand the relationship of those services to the individual\n\nguardianship plan for the management of financial resources;\n\n3. Any significant actions taken by the guardian or limited\n\nguardian during the reporting period;\n\n4. Any significant problems relating to the guardianship which\n\nhave arisen during the reporting period; and\n\n5. If the ward is an incapacitated or partially incapacitated\n\nperson, the reasons, if any, why the guardianship should not be\n\nterminated, or why no less restrictive alternative would permit the\n\nward to manage his financial resources.\n\nB. If the ward is an incapacitated or partially incapacitated\n\nperson, reports on the guardianship of the property shall be\n\nsubstantially in the following form:\n\nReport on the Guardianship of Property\n\nI, (Name) the (Guardian or Limited Guardian of the\n\nproperty) of (Name) , an incapacitated (or a partially\n\nincapacitated) person, hereby submit this (annual, court-ordered)\n\nReport.\n\n1. List any significant changes in the capacity of the ward to\n\nmanage his or her financial resources:__________________________\n\n________________________________________________________________\n\n2. The services currently being provided to the ward are as\n\nfollows:\n\n_________________________________________________________\n\n3. These services (are, are not) provided for in the current\n\nGuardianship Plan as approved by the court.\n\nThe reason these services are not shown in the current plan are as\n\nfollows:_______________________________________________________\n\n_______________________________________________________________\n\n4. I (have, have not) taken any significant actions for or on\n\nbehalf of the ward since the last time I submitted a Guardianship\n\nReport.\n\n(If so:) These actions are as follows:\n\n_______________________________________________________________\n\n5. There (have, have not) been any significant problems relating to\n\nthe guardianship since the last time I submitted a Guardianship\n\nReport.\n\n(If so:) The problems are as follows:\n\n_______________________________________________________________\n\n6. In my opinion, the guardianship (should, should not) be\n\ncontinued. The reasons for my belief are as follows:\n\n_______________________________________________________________\n\n7. It is my belief that the ward (would, would not) be able to\n\nmanage his or her financial resources with fewer restrictions on the\n\nward's ability to act for him or herself. Whenever there are changes or proposed changes to the\n\nguardianship plan, an individual guardianship plan for the\n\nmanagement of financial resources, substantially in the same form as\n\nprovided in Section 3-122 of this title, shall be submitted with the\n\nguardianship report and shall show any such changes or proposed\n\nchanges in the guardianship plan since last submitted and approved\n\nby the court.\n\nE. The report:\n\n1. Shall contain a complete financial statement of the\n\nfinancial resources of the ward under the control or supervision of\n\nthe guardian or limited guardian of the property;\n\n2. Shall contain an accounting of any receipts and\n\ndisbursements received, or expenditures made by the guardian or\n\nlimited guardian on behalf of the ward;\n\n3. May include any request for compensation for the guardian;\n\nand\n\n4. May include any compensation request for the attorney for\n\nthe ward.\n\nF.
