The Secretary\n\nof State shall cause to be affixed onto the back of the signature\n\nform an affidavit, subject to change, for the circulator to verify\n\nthe signatures.\n\nState of Oklahoma, )\n\n) ss.\n\nCounty of ________ )\n\nI, ______, being first duly sworn, say: That I am at least\n\neighteen (18) years old, a registered voter of this state, and that\n\nall signatures on the signature sheet were signed in my presence; I\n\nbelieve that each has stated his or her name, mailing address,\n\ncounty of residence, and date of birth associated with his or her\n\nOklahoma voter registration record, and that each signer is a legal\n\nvoter of the State of Oklahoma and county of ______ or of the city\n\nof ______ (as the case may be).
