All applications shall be accompanied by the applicable\n\nfees. An appointment may be deemed by the Commissioner to have\n\nterminated upon failure by the insurer to pay the prescribed renewal\n\nfee. The Commissioner may also by order impose a civil penalty\n\nequal to double the amount of the unpaid renewal fee.\n\nThe Insurance Commissioner shall collect in advance the\n\nfollowing fees and licenses:\n\n1. For filing appointment of Insurance\n\n2. Miscellaneous:\n\na. Insurance producer's study manual:\n\n$ 40.00\n\n$ 40.00\n\nb. For filing organizational documents of\n\nan entity applying for a license as an\n\n3. Examination for license:\n\nFor each examination covering laws\n\nand one or more lines of insurance.... not to exceed\n\n$100.00\n\n4. Licenses:\n\nb. Nonresident insurance producer's\n\nc. Insurance producer's biennial license\n\nfor sale or solicitation of variable\n\ne. Nonresident limited lines producer\n\nf. (1) Car rental limited lines biennial\n\nlicense, one or two locations,\n\n(2) Car rental limited lines biennial\n\nlicense, three or more locations,\n\nh. Managing general agent's biennial\n\nj. Insurance vending machine, each machine,\n\n5. Annual fee for each appointed insurance\n\nproducer, managing general agent, or limited\n\nlines producer by insurer, each license of\n\n6. Renewal fee for all licenses shall be the same as the\n\ncurrent initial license fee.\n\n7. The fee for a duplicate license shall be one-half (1/2) the\n\nfee of an original license.\n\n8. The renewal of a license shall require a fee of double the\n\ncurrent original license fee if the application for renewal is late,\n\nor incomplete on the renewal deadline.\n\n9. The administrative fee for submission of a change of legal\n\nname or address more than thirty (30) days after the change occurred\n\nshall be Fifty Dollars ($50.00).\n\nB. If for any reason an insurance producer license or\n\nappointment is not issued or renewed by the Commissioner, all fees\n\naccompanying the appointment or application for the license shall be\n\ndeemed earned and shall not be refundable except as provided in\n\nSection 352 of this title.\n\nC.
