An insurer shall provide to any insurance producer\n\nauthorized to sell life, accident or health insurance products,\n\nwhose appointment has been terminated for any reason other than the\n\nreasons set forth in Section 1435.13 of this title, information\n\nrelating to the policy of the person who purchased a product from\n\nsuch producer if the insured has signed a form authorizing the\n\nrelease of the information.\n\nB. The Insurance Commissioner shall prescribe the form required\n\nby subsection A of this section. The form shall be in compliance\n\nwith federal and state laws and regulations relating to privacy.\n\nC.
