Stat. tit. 36, § 36-2412","heading":"Medical examination of applicant; warranties and","body":"certificates in lieu of examination; concealment or\n\nmisrepresentation.\n\nA. Applicants for membership in mutual benefit associations\n\nshall be required to pass a medical or physical examination, if\n\nrequired by the bylaws of the association; or benefit certificates\n\nmay be issued upon the warranty by the applicant that the answers\n\nand statements to the questions as to the condition of health of the\n\napplicant and all statements and answers made or appearing in the\n\napplication for a benefit certificate are true and are to be used as\n\nthe basis and consideration upon which said benefit certificate is\n\nissued, provided, that in addition to the answers and\n\nrepresentations of the applicant for membership, as herein provided,\n\nthe applicant shall give the name and address of his or her family\n\nphysician and shall if required by the association furnish to the\n\nassociation a certificate of such physician, or if the applicant has\n\nno family physician, a certificate of a licensed physician, that he\n\nknows the applicant and believes that the applicant is a good,\n\ninsurable risk; provided, that if the family physician refuses to\n\nsign a certificate stating whether applicant is a good insurable\n\nrisk, a certificate from some other licensed physician may be\n\nsecured; and, provided, further, that the fee for such certificate\n\nshall not exceed fifty cents ($0.50).\n\nB. Any beneficiary, his agent or representative, under any\n\npolicy or certificate issued by any association or company coming\n\nwithin this article, or any doctor, undertaker or other attendant,\n\nwho shall knowingly conceal, withhold, or misrepresent any facts in\n\nany verified report or declaration under oath concerning the health,\n\nage, cause of death or other material information as to the deceased\n\nmember or policy holder because of whose death or accident claim is\n\nbeing made, shall be guilty of perjury and shall be subject to the\n\npenalties therefor prescribed by law.","path":["OK Code","Title 36"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os36.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"96d77731233ba8d907d87ec8887f32e934fe9347e95405db3efe9e632a287463","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-36-36-2411","next":"us-ok/okla.-stat.-tit.-36-36-2413"},"notice":"GroundRules: Original legal text.
