Stat. tit. 36, § 36-2652","heading":"Application for certificate - Contents - Fee","body":"Such a corporation may issue contracts to its subscribers only\n\nwhen the Insurance Commissioner has, by certificate of authority,\n\nauthorized it so to do. Application for such certificate of\n\nauthority shall be made on forms supplied or approved by the\n\nCommissioner, containing such information as he shall deem\n\nnecessary. Each application for such certificate of authority shall\n\nbe accompanied by the fee prescribed by Section 321 of the Oklahoma\n\nInsurance Code, and copies of the following documents:\n\n1. Articles of Incorporation signed by not less than fifty\n\noptometrists licensed to practice in Oklahoma;\n\n2. Bylaws;\n\n3. Proposed contracts, if any, between the applicant and\n\nparticipating optometrists, showing the terms under which service is\n\nto be furnished to subscribers;\n\n4. Proposed contracts to be issued to subscribers;\n\n5. A table of rates to be charged to subscribers;\n\n6. Financial statement of the corporation, including the\n\namounts of contributions paid or agreed to be paid to the\n\ncorporation for working capital and the name or names of each\n\ncontributor and the terms of each contribution; and\n\n7.
