Stat. tit. 36, § 36-2656","heading":"Filing of forms and rates; disapproval","body":"On and after the effective date of this act, no contract\n\nproviding for optometric service or indemnity shall be issued or\n\ndelivered to any person in this state, nor shall any application,\n\nrider or endorsement be issued in connection therewith, until a copy\n\nof the form thereof, and the rates pertaining thereto, have been\n\nfiled with and approved by the Insurance Commissioner. If the\n\nInsurance Commissioner disapproves the contract, application, rider\n\nor endorsement form, or rates, said Commissioner shall make a\n\nwritten decision stating the reason or reasons therefor and shall\n\ndeliver a copy thereof to the corporation and it shall be unlawful\n\nfor any such corporation to use any such form in this state.
