Stat. tit. 36, § 36-2672","heading":"Application for certificate - Contents - Fee","body":"A dental service corporation may issue contracts to its\n\nsubscribers only when the Insurance Commissioner has, by certificate\n\nof authority, authorized it to do so. Application for such\n\ncertificate of authority shall be made on forms supplied or approved\n\nby the Commissioner, containing such information as he shall deem\n\nnecessary. Each application for such certificate of authority shall\n\nbe accompanied by the fee prescribed by Article 3 of Title 36,\n\nOklahoma Statutes, and copies of the following documents:\n\n1. Articles of Incorporation;\n\n2. Bylaws;\n\n3. Proposed contracts to be issued to subscribers;\n\n4. Financial statement of the corporation, including the\n\namounts of contributions paid; and\n\n5.
