Stat. tit. 36, § 36-2691.6","heading":"Filing of forms and rates; disapproval","body":"On and after the effective date of this act, no contract\n\nproviding for chiropractic service or indemnity shall be issued or\n\ndelivered to any person in this state, nor shall any application,\n\nrider or endorsement be issued in connection therewith, until a copy\n\nof the form thereof and the rates pertaining thereto have been filed\n\nwith and approved by the Insurance Commissioner. If the Insurance\n\nCommissioner disapproves the contract, application, rider or\n\nendorsement form, or rates, he shall make a written decision stating\n\nthe reason or reasons therefor and shall deliver a copy thereof to\n\nthe corporation and it shall be unlawful for any such corporation to\n\nuse any such form in this state.
