No service contract shall be executed, issued or delivered\n\nin this state until the form thereof has been approved in writing by\n\nthe Commissioner, and all promotional and advertising material,\n\nmembership cards and other indicia of membership shall be submitted\n\nfor approval on the request of the Commissioner.\n\nB. Every service contract executed, issued or delivered in this\n\nstate shall be made in duplicate, with one copy being kept by the\n\nissuing company and the other copy delivered to the purchasing\n\nparty.\n\nC. No service contract shall be executed, issued or delivered\n\nin this state unless it contains the following:\n\n1. The exact corporate or other name of the company;\n\n2. The exact location of its home office and of its usual place\n\nof business in this state, giving street number and city;\n\n3. A provision that the contract may be canceled at any time by\n\nthe club or canceled at any time by the holder, if the club or its\n\nagent have violated any of the provisions of Section 3 or Section 5\n\nof this act in soliciting the purchase of such contract from the\n\nholder. If the contract is canceled, pursuant to this provision,\n\nthe holder will, if he has actually paid the consideration,\n\nthereupon be entitled to the unused portion of the consideration\n\npaid for such contract, calculated on a pro rata basis over the\n\nperiod of the contract, without any deductions.\n\n4. Provisions plainly specifying:\n\na. the services promised,\n\nb. that the holder will not be required to pay any sum, in\n\naddition to the amount specified in the contract, for any services\n\nthus specified,\n\nc. the territory wherein such services are to be rendered,\n\nand\n\nd. the date when such service will commence.\n\n5.
