All health benefit plans which provide coverage for a family\n\nmember of the insured or subscriber shall offer coverage for child\n\nhealth supervision services. Such services shall include coverage\n\nfrom the moment of birth through the age of eighteen years. Each\n\nsuch plan or contract shall, at a minimum, provide benefits for\n\nchild health supervision services at approximately the following age\n\nintervals: birth, two months, four months, six months, nine months,\n\ntwelve months, eighteen months, two years, three years, four years,\n\nfive years, six years, eight years, ten years, twelve years,\n\nfourteen years, sixteen years and eighteen years. A health benefit\n\nplan may provide that child health supervision services which are\n\nrendered during a periodic review shall only be covered to the\n\nextent that services are provided by or under the supervision of a\n\nsingle physician or other primary health care provider during the\n\ncourse of one visit. Benefits for such services shall be subject to\n\nthe same durational limits, dollar limits, deductibles and\n\ncoinsurance factors as other covered services in such health\n\ninsurance policies. All Oklahoma health benefit plans delivered,\n\nissued for delivery, modified or renewed on or after January 1,\n\n1995, shall be subject to the provisions of this section.\n\nB.
