The Insurance Commissioner shall collect in advance the\n\nfollowing fees:\n\n1. For filing charter documents:\n\nOriginal charter documents,\n\narticles of incorporation, bylaws,\n\nor record of organization of alien\n\nor foreign insurers, or certified\n\n2. Certificate of Authority or Certificate of Approval:\n\n(a) Issuance:\n\n(b) Renewal:\n\n3. For filing appointment of Insurance\n\nCommissioner as agent for service\n\n4. Miscellaneous:\n\n(b) Amended charter documents,\n\narticles of incorporation or\n\nbylaws of domestic, alien or\n\nforeign insurers or health\n\n(c) Certificate of Commissioner,\n\n(d) For filing Merger and\n\nAcquisition Forms (Domestic\n\n(e) For filing Variable Product\n\n(f) For filing a Life, Accident\n\nand Health Policy and Health\n\nMaintenance Organization\n\n(g) For filing an advertisement or\n\nrider application to a Life,\n\nAccident and Health Policy and\n\nHealth Maintenance\n\n(i) For filing a Viatical\n\nSettlement Contract or Life\n\n(j) For filing an advertisement\n\nfor Viatical Settlement or\n\n(k) For filing application for\n\nViatical Settlement or Life\n\nB.
