Stat. tit. 36, § 36-3607","heading":"Application required","body":"No life or accident and health insurance contract upon an\n\nindividual, except a contract of group life insurance or of group or\n\nblanket accident and health insurance, shall be made or effectuated\n\nunless at the time of the making of the contract the individual\n\ninsured, being of competent legal capacity to contract, applies\n\ntherefor or consents thereto, except in the following cases:\n\n1. A spouse may effectuate such insurance upon the other\n\nspouse.\n\n2. Any person having an insurable interest in the life of a\n\nminor, or any person upon whom a minor is dependent for support and\n\nmaintenance, may effectuate insurance upon the life of or pertaining\n\nto such minor.\n\n3.
