No application for the issuance of any life insurance policy\n\nor contract shall be admissible in evidence in any action relative\n\nto such policy or contract, unless a true copy of the application\n\nwas attached to or otherwise made a part of the policy when issued.\n\nThis provision shall not apply to industrial life insurance\n\npolicies.\n\nB. If any policy of life insurance delivered in this state is\n\nreinstated or renewed, and the insured or the beneficiary or\n\nassignee of the policy makes written request to the insurer for a\n\ncopy of the application, if any, for such reinstatement or renewal,\n\nthe insurer shall, within thirty (30) days after receipt of such\n\nrequest at its home office or at any of its branch offices, deliver\n\nor mail to the person making such request a copy of such\n\napplication. If such copy is not so delivered or mailed after\n\nhaving been so requested, the insurer shall be precluded from\n\nintroducing the application in evidence in any action or proceeding\n\nbased upon or involving the policy or its reinstatement or renewal.\n\nIn the case of such a request from a beneficiary or assignee, the\n\ntime within which the insurer is required to furnish a copy of such\n\napplication shall not begin to run until after receipt of evidence\n\nsatisfactory to the insurer of the beneficiary's or assignee's\n\nvested interest in the policy or contract.\n\nC. As to kinds of insurance other than life insurance, no\n\napplication for insurance signed by or on behalf of the insured\n\nshall be admissible in evidence in any action between the insured\n\nand the insurer arising out of the policy so applied for, if the\n\ninsurer has failed, at expiration of thirty (30) days after receipt\n\nby the insurer of written demand therefor by or on behalf of the\n\ninsured, to furnish to the insured a copy of such application\n\nreproduced by any legible means.","path":["OK Code","Title 36"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os36.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"46856f113453d400412f17f932981c037fc746478504260af9357d1fed4b66bc","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-36-36-3607","next":"us-ok/okla.-stat.-tit.-36-36-3609"},"notice":"GroundRules: Original legal text.
