A health benefit plan that provides benefits for\n\nprescription drugs delivered, issued or renewed after November 1,\n\n2017, shall provide for synchronization of prescription drug refills\n\non at least one occasion per insured per year, provided all of the\n\nfollowing conditions are met:\n\n1. The prescription drugs are covered by the health benefit\n\nplan's clinical coverage policy or have been approved by a formulary\n\nexceptions process;\n\n2. The prescription drugs are maintenance medications as\n\ndefined by the plan and have available refill quantities at the time\n\nof synchronization;\n\n3. The medications are not Schedule II, III or IV controlled\n\nsubstances;\n\n4. The insured meets all utilization management criteria to the\n\nprescription drugs at the time of synchronization;\n\n5. The prescription drugs are of a formulation that can be\n\nsafely split into short-fill periods to achieve synchronization;\n\n6. The prescription drugs do not have special handling or\n\nsourcing needs as determined by the plan, contract, or agreement\n\nthat require a single, designated pharmacy to fill or refill the\n\nprescription; and\n\n7. The covered person agrees to the synchronization.\n\nB. When necessary to permit synchronization, the health benefit\n\nplan shall apply a prorated daily cost-sharing rate to any\n\nmedication dispensed by a network pharmacy pursuant to this section.\n\nNo dispensing fees shall be prorated, and all dispensing fees shall\n\nbe based on the number of prescriptions filled or refilled.\n\nC.
