The Life, Accident and Health Insurance Policy Language\n\nSimplification Act shall apply to all policies delivered or issued\n\nfor delivery in this state by any company on or after the date such\n\nforms must be approved, except:\n\n1. any policy which is a security subject to federal\n\njurisdiction;\n\n2. any group annuity contract which serves as a funding\n\nmechanism for pension, profit sharing, or deferred compensation\n\nplans;\n\n3. any form used in connection with, as a conversion from, as\n\nan addition to, or in exchange pursuant to a contractual provision\n\nfor, a policy delivered or issued for delivery on a form approved or\n\npermitted to be issued prior to January 1, 1990;\n\n4. the renewal of a policy delivered or issued for delivery\n\nprior to January 1, 1990; or\n\n5. any group policy covering a group of one thousand or more\n\nlives at the date of issue, other than a group credit life insurance\n\npolicy or a group credit health insurance policy; however, this\n\nshall not exempt any certificate issued pursuant to a group policy\n\ndelivered or issued for delivery in this state.\n\nB.
