Stat. tit. 36, § 36-4402","heading":"Accident and health policies; filing","body":"On and after the effective date of this Code no policy of\n\ninsurance against loss or expense from the sickness, or from the\n\nbodily injury or death of the insured by accident shall be issued or\n\ndelivered to any person in this state, nor shall any application,\n\nrider or endorsement be used in connection therewith until a copy of\n\nthe form thereof, and of the classification of risks, and the\n\npremium rates pertaining thereto, have been filed with the Insurance\n\nCommissioner. If the Insurance Commissioner disapproves the policy,\n\napplication, rider or endorsement form, said Commissioner shall make\n\na written decision stating the reason or reasons therefor, and shall\n\ndeliver a copy thereof to the company, and it shall be unlawful for\n\nany such insurer to use any such form in the state.
