Stat. tit. 36, § 36-4410","heading":"Franchise accident and health insurance law","body":"Accident and health insurance on a franchise plan is hereby\n\ndeclared to be that form of accident and health insurance issued to:\n\n1. Five or more employees of any corporation, copartnership or\n\nindividual employer or any governmental corporation, agency or\n\ndepartment thereof; or\n\n2. Ten or more members, employees or employees of members of\n\nany trade or professional association or of a labor union or of any\n\nother association having had an active existence for at least two\n\n(2) years where such association or union has a constitution or\n\nbylaws and is formed in good faith for purposes other than that of\n\nobtaining insurance;\n\nwhere such persons, with or without their dependents, are issued the\n\nsame form of an individual policy varying only as to amounts and\n\nkinds of coverage applied for by such persons under an arrangement\n\nwhereby the premiums on such policies may be paid to the insurer\n\nperiodically by the employer, with or without payroll deductions, or\n\nby the association for its members, or by some designated person\n\nacting on behalf of such employer or association.
