Blanket accident and health insurance is hereby declared to\n\nbe that form of accident and health insurance covering special\n\ngroups of persons as enumerated in one of the following paragraphs\n\n(1) to (7) inclusive:\n\n1. Under a policy or contract issued to any common carrier,\n\nwhich shall be deemed the policyholder, covering a group defined as\n\nall persons who may become passengers on such common carrier.\n\n2. Under a policy or contract issued to an employer, who shall\n\nbe deemed the policyholder, covering all employees or any group of\n\nemployees defined by reference to exceptional hazards incident to\n\nsuch employment. Dependents of the employees and guests of the\n\nemployer may also be included where exposed to the same hazards.\n\n3. Under a policy or contract issued to a college, school, or\n\nother institution of learning or to the head or principal thereof,\n\nwho or which shall be deemed the policyholder, covering students or\n\nteachers.\n\n4. Under a policy or contract issued in the name of any\n\nvolunteer fire department, first aid, or other such volunteer group,\n\nor agency having jurisdiction thereof, which shall be deemed the\n\npolicyholder, covering all of the members of such fire department or\n\ngroup.\n\n5. Under a policy or contract issued to a creditor, who shall\n\nbe deemed the policyholder, to insure debtors of the creditor.\n\n6. Under a policy or contract issued to a sports team or to a\n\ncamp or sponsor thereof, which shall be deemed the policyholder,\n\ncovering members or campers.\n\n7. Under a policy or contract issued to any other substantially\n\nsimilar group which, in the discretion of the Insurance\n\nCommissioner, may be subject to the issuance of a blanket accident\n\nand health policy or contract.\n\nB. An individual application need not be required from a person\n\ncovered under a blanket accident and health policy or contract, nor\n\nshall it be necessary for the insurer to furnish each person a\n\ncertificate.\n\nC. All benefits under any blanket accident and health policy\n\nshall be payable to the person insured, or to his designated\n\nbeneficiary or beneficiaries, or to his estate, except that if the\n\nperson insured be a minor, such benefits may be made payable to his\n\nparent, guardian, or other person actually supporting him; except,\n\nthat the policy may provide that subject to any written direction of\n\nthe insured in the application or otherwise, all or any portion of\n\nany indemnities provided by any such policy on account of hospital,\n\nnursing, medical or surgical services may, at the insurer's option\n\nand unless the insured requests otherwise not later than the time of\n\nfiling proofs of such loss, be paid directly to the hospital or\n\nperson rendering such services, but the policy may not require that\n\nthe service be rendered by a particular hospital or person. Payments\n\nso made shall discharge the insurer's obligation with respect to the\n\namount of insurance so paid.\n\nD.
