Each Health Insurance Purchasing Group (HIPG) shall file\n\nforms as may be described by rules and regulations of the\n\nCommissioner.\n\nB. Each HIPG health carrier shall file the health benefits plan\n\nto be issued to a HIPG pursuant to Article 36 of the Oklahoma\n\nInsurance Code.\n\nC. Each HIPG health carrier, which develops or offers a health\n\nbenefits plan for a HIPG that is a limited benefit plan not subject\n\nto state-mandated health benefits, shall specify on the face page of\n\nthe policy and certificate, printed in ten-point or larger type, a\n\nstatement that clearly indicates in substance the following:\n\n“IMPORTANT NOTICE: This policy is a limited benefit contract\n\nwhich has been established by a Health Insurance Purchasing Group\n\n(HIPG). It may not contain mandated benefits found under Oklahoma\n\nInsurance Laws.
