Any provision, exclusion or limitation in an accident and\n\nhealth insurance policy which:\n\n1. Denies an insured, or the insured's parent or guardian if\n\nthe insured is a minor, the free choice of any practitioner or the\n\nuse of any hospital, home care agency or ambulatory surgical center\n\nwhere the practitioner is authorized to practice, doctor's office or\n\nclinic; or\n\n2. Otherwise conflicts with any provision of the Health Care\n\nFreedom of Choice Act,\n\nshall, to the extent of the denial or conflict, be void, but such\n\nvoidance shall not affect the validity of the other provisions of\n\nthe policy.\n\nB.
