All individual and group health insurance policies providing\n\ncoverage on an expense incurred basis, and all individual and group\n\nservice or indemnity type contracts issued by a nonprofit\n\ncorporation which provide coverage for a female forty-five (45)\n\nyears of age or older in this state, except for policies that\n\nprovide coverage for specified disease or other limited benefit\n\ncoverage, shall include the coverage specified by this section for a\n\nbone density test to qualified individuals covered by the policy\n\nwhen such test is requested by a primary care or referral physician.\n\nThe test shall be subject to the policy deductible, copayments and\n\ncoinsurance limits of the plan; provided, however, no policy or\n\ncontract shall be required to reimburse more than One Hundred Fifty\n\nDollars ($150.00) for any such test.\n\nB. For purposes of this section:\n\n1. \"Qualified individual\" means an individual:\n\na. with an estrogen hormone deficiency,\n\nb. with:\n\n(1) vertebral abnormalities,\n\n(2) primary hyperparathyroidism, or\n\n(3) a history of fragility bone fractures,\n\nc. who is receiving long-term glucocorticoid, or\n\nd. who is currently under treatment for osteoporosis; and\n\n2. \"Bone density test\" means a medically accepted measurement\n\nof bone mass used to detect low bone mass and to determine a\n\nqualified individual's risk for osteoporosis.","path":["OK Code","Title 36"],"source_url":"https://www.oklegislature.gov/OK_Statutes/CompleteTitles/os36.pdf","current_through":"2026-08-14","vintage":"open-us-law v2026.08, retrieved 2026-09-14","retrieved_at":"2026-09-14T18:32:36Z","sha256":"ce08681a6e443ba8a0e2de346cfd11a4836c7ff1e4ee762f30344eba1a034df5","source_id":"us-ok","stale":false,"prev":"us-ok/okla.-stat.-tit.-36-36-6060","next":"us-ok/okla.-stat.-tit.-36-36-6060.10"},"notice":"GroundRules: Original legal text.
