Stat. tit. 36, § 36-6060.51","heading":"Definitions","body":"As used in this section:\n\n1. “Health benefit plan” means group hospital coverage,\n\nindividual and group medical insurance coverage, a not-for-profit\n\nhospital or medical service or indemnity plan, a prepaid health\n\nplan, a health maintenance organization plan, a preferred provider\n\norganization plan, the Oklahoma Employees Insurance Plan, and\n\ncoverage provided by a multiple employer welfare arrangement. The\n\nterm shall not include:\n\na. a plan that provides coverage:\n\n(1) only for a specified disease or diseases or under\n\nan individual limited benefit policy,\n\n(2) only for accidental death or dismemberment,\n\n(3) only for dental or vision care,\n\n(4) for a hospital confinement indemnity policy,\n\n(5) for disability income insurance or a combination\n\nof accident-only and disability income insurance,\n\nor\n\n(6) as a supplement to liability insurance,\n\nb. any health plan offered by a contracted entity, as\n\ndefined in Section 4002.2 of Title 56 of the Oklahoma\n\nStatutes, that provides coverage to members of the\n\nstate Medicaid program,\n\nc. a Medicare supplemental policy as defined by Section\n\n1882(g)(1) of the Social Security Act (42 U.S.C.,\n\nSection 1395ss),\n\nd. workers’ compensation insurance coverage,\n\ne. medical payment insurance issued as part of a motor\n\nvehicle insurance policy,\n\nf. a long-term care policy, including a nursing home\n\nfixed indemnity policy, unless a determination is made\n\nthat the policy provides benefit coverage so\n\ncomprehensive that the policy meets the definition of\n\na health benefit plan, or\n\ng. short-term health insurance issued on a nonrenewable\n\nbasis with a duration of six (6) months or less;\n\n2. “Health care provider” means the same as defined in Section\n\n1219.6 of Title 36 of the Oklahoma Statutes; and\n\n3. “Health care service” means any service provided by a health\n\ncare provider, or by an individual working for or under the\n\nsupervision of a health care provider, that relates to the\n\ndiagnosis, assessment, prevention, treatment, or care of any human\n\nillness, disease, injury, or condition.\n\nThe term shall also include mental health and substance use\n\ndisorder services, as defined by Section 6060.10 of Title 36 of the\n\nOklahoma Statutes, and durable medical equipment as defined by\n\nSection 375.2 of Title 59 of the Oklahoma Statutes.
