Any health benefit plan that is offered, issued or renewed\n\nin this state on or after January 1, 2000, that provides coverage to\n\nmen forty (40) years of age or older in this state shall offer\n\ncoverage for annual screening for the early detection of prostate\n\ncancer in men over the age of fifty (50) years and in men over the\n\nage of forty (40) years who are in high-risk categories. The\n\ncoverage shall not be subject to policy deductibles. The coverage\n\nshall not exceed the actual cost of the prostate cancer screening up\n\nto a maximum of Sixty-five Dollars ($65.00) per screening.\n\nB. The benefit required to be provided by subsection A of this\n\nsection shall in no way diminish or limit diagnostic benefits\n\notherwise allowable under a health benefit plan.\n\nC. The prostate cancer screening coverage shall be offered as\n\nfollows:\n\n1. The screening shall be performed by a qualified medical\n\nprofessional including, but not limited to, a urologist, internist,\n\ngeneral practitioner, doctor of osteopathy, nurse practitioner, or\n\nphysician assistant;\n\n2. The screening shall consist, at a minimum, of the following\n\ntests:\n\na. a prostate-specific antigen blood test, and\n\nb. a digital rectal examination;\n\n3. At least one screening per year shall be covered for any man\n\nfifty (50) years of age or older; and\n\n4. At least one screening per year shall be covered for any man\n\nfrom forty (40) to fifty (50) years of age who is at increased risk\n\nof developing prostate cancer as determined by a physician.\n\nD.
