Any health benefit plan, including the State and Education\n\nEmployees Group Health Insurance Plan, that is offered, issued, or\n\nrenewed in this state on or after January 1, 2001, that provides\n\nmedical and surgical benefits with respect to the treatment of\n\ncancer and other conditions treated by chemotherapy or radiation\n\ntherapy shall provide coverage for wigs or other scalp prostheses\n\nnecessary for the comfort and dignity of the covered person.\n\nB. The coverage provided for by this section shall be subject\n\nto the same annual deductibles, copayments, or coinsurance limits as\n\nestablished for all other covered benefits under the health benefit\n\nplan not to exceed One Hundred Fifty Dollars ($150.00) annually.\n\nC. A health benefit plan shall provide notice to each insured\n\nor enrollee under the plan regarding the coverage required by this\n\nsection in the evidence of coverage of the plan and shall provide\n\nadditional written notice of the coverage to the insured or enrollee\n\nas follows:\n\n1. In the next mailing made by the plan to the insured or\n\nenrolled employee;\n\n2. As part of any yearly informational packet sent to the\n\nenrollee; or\n\n3. Not later than December 1, 2000;\n\nwhichever is earlier.\n\nD. As used in this act, \"health benefit plan\" means any plan or\n\narrangement as defined in subsection C of Section 6060.4 of this\n\ntitle. However, this section shall not apply to policies or\n\ncertificates issued to individuals or groups with fifty (50) or\n\nfewer employees or plans offered under the State Medicaid Program.\n\nE.
