Stat. tit. 36, § 36-6124.2","heading":"Application for change of name of a prepaid funeral","body":"benefit permit holder.\n\nA. No organization shall change the name under which it sells\n\nprepaid funeral benefits except as provided in this section. The\n\norganization shall obtain approval from the Insurance Commissioner\n\nat least thirty (30) days prior to changing the name of the permit\n\nholder. The application for change of name shall be in the manner\n\nand form prescribed by the Insurance Commissioner and shall contain,\n\nat a minimum, the following information:\n\n1. The name of the organization as it currently appears on its\n\npermit;\n\n2. The proposed new name of the organization;\n\n3. The proposed date the name change will become effective;\n\n4. The business address of the organization; and\n\n5. The organization's primary email address.\n\nB. The Insurance Commissioner may deny the organization's\n\nchange of name upon good cause shown.\n\nC. Upon approval of a change of name, the Insurance\n\nCommissioner shall issue a prepaid funeral benefit permit with the\n\nnew name.\n\nD. Every organization shall display in a conspicuous place at\n\nall times on the premises of the organization all permits issued\n\npursuant to the provisions of this act. No organization may consent\n\nto or allow the use or display of the permit by a person other than\n\nthe persons authorized to represent the organization in contracting\n\nprepaid funeral benefits.\n\nE. The Insurance Commissioner may promulgate rules concerning\n\nmatters incidental to this section.\n\nF.
