Stat. tit. 36, § 36-6128","heading":"Annual report","body":"Each organization shall file an annual report with the Insurance\n\nCommissioner on or before March 15 of each year in such form as the\n\nInsurance Commissioner may require, showing the names and addresses\n\nof all persons with whom contracts for prepaid funeral benefits have\n\nbeen made prior to December 31 of the preceding year which had not\n\nbeen fully discharged on December 31 and, also showing the date of\n\nthe contract, the name of the bank or depository holding the trust\n\nfund and the amount of the trust fund under each contract on the\n\npreceding December 31. Any organization which has discontinued the\n\nsale of prepaid funeral benefits, but which still has outstanding\n\ncontracts, shall not be required to obtain a renewal of its permit,\n\nbut it shall continue to make annual reports to the Insurance\n\nCommissioner until all such contracts have been fully discharged. A\n\nfiling fee of Fifty Dollars ($50.00), along with any transaction or\n\nother applicable fees, shall accompany each report.
