Each year on or before March 1, every prepaid dental plan\n\norganization shallfile with the Commissioner a report of the\n\nbusiness activities of said organization for the preceding calendar\n\nyear. This report shall be signed by at least two principal\n\nofficers of the corporation and said signatures shall be notarized.\n\nB. Said reports shall be on forms prescribed by the\n\nCommissioner and shall include:\n\n1. A financial statement of the organization, including a copy\n\nof the balance sheet and receipts and disbursements of the\n\norganization for the preceding year certified by an independent\n\ncertified public accountant. The Commissioner may accept a full\n\nreport of the most recent examination of a foreign prepaid dental\n\nplan, certified to by the appropriate examining official of another\n\nstate; and\n\n2. Any material changes in the information required to be\n\nprovided pursuant to Section 5 of this act; and\n\n3. The number of persons who have become members during the\n\npreceding year, the total number of members in the plan as of the\n\nend of the year, and the number of memberships terminated during the\n\nyear; and\n\n4. The costs of all care provided and the number of members who\n\nreceived care pursuant to the provisions of the prepaid dental plan;\n\nand\n\n5.
