Stat. tit. 36, § 36-615.2","heading":"Biographical affidavit","body":"All domestic insurers and health maintenance organizations are\n\nrequired to keep biographical information current. Domestic\n\ninsurers and health maintenance organizations are required to\n\nprovide Biographical Affidavits within thirty (30) days of any\n\nchange in officers, directors, key management or any person\n\nacquiring ten percent (10%) or more controlling interest in a\n\ndomestic insurer. The information shall be on the National\n\nAssociation of Insurance Commissioners (NAIC) UCAA Biographical\n\nAffidavit Form. The Biographical Affidavit is to be certified by an\n\nindependent third party acceptable to the Insurance Commissioner\n\nthat has conducted a comprehensive review of the background of the\n\napplicant and has indicated that the Biographical Affidavit has no\n\nsignificantly inaccurate or conflicting information and is accepted\n\nas the Business Character Report. As used in this section,\n\n\"independent third party\" is one that has no affiliation with the\n\napplicant and is in the business of providing background checks or\n\ninvestigations.
