Stat. tit. 36, § 36-6211","heading":"Form of license - Contents","body":"The license of an adjuster shall be in a form prescribed by the\n\nInsurance Commissioner. The license shall contain:\n\n1. The name and mailing address of the adjuster;\n\n2. Indication as to whether the adjuster is licensed as an\n\ninsurance adjuster or as a public adjuster;\n\n3. The date of issuance and the date of expiration of the\n\nlicense;\n\n4. The classes of business the license is to cover; and\n\n5.
