Stat. tit. 36, § 36-6212","heading":"Fees - Notification of change of name, address, or e-mail","body":"address.\n\nA. The Insurance Commissioner or an administrator approved by\n\nthe Insurance Commissioner shall collect a fee of Twenty Dollars\n\n($20.00) for an examination for an adjuster's license in any of the\n\nfollowing single classes of business. The fee for any examination\n\nwhich includes two or more classes of business shall not exceed\n\nForty Dollars ($40.00). The classes of business are:\n\n1. Motor vehicle physical damage;\n\n2. Fire and allied lines (property);\n\n3. Casualty;\n\n4. Workers' compensation;\n\n5. Crime and fidelity bonds; and\n\n6. Crop/hail.\n\nB. The Commissioner shall collect the following fees for an\n\nadjuster's license:\n\n1. For a license in any single class of business, every two (2)\n\nyears, Thirty Dollars ($30.00);\n\n2. For a license in any combination of two or more classes of\n\nbusiness, every two years, Fifty Dollars ($50.00);\n\n3. Public adjuster, every two years, Thirty Dollars ($30.00);\n\n4. Emergency adjuster, as provided for in Section 6218 of this\n\ntitle, each year, Fifteen Dollars ($15.00); and\n\n5. Apprentice adjuster, as provided for in Section 6204.1 of\n\nthis title, Twenty Dollars ($20.00).\n\nC. The fees prescribed in this section shall accompany the\n\napplication for an original license or a renewal of a license.\n\nD. The fee for the original license or renewal license shall be\n\ncollected in advance of issuance. Late application for renewal\n\nshall require a fee of double the amount of the original license\n\nfee.\n\nE. The Commissioner may issue a duplicate license for any lost,\n\nstolen, or destroyed license issued pursuant to the provisions of\n\nthe Insurance Adjusters Licensing Act if an affidavit is submitted\n\nby the licensee to the Commissioner concerning the facts of such\n\nloss, theft, or destruction. The affidavit shall be in a form\n\nprescribed by the Commissioner. The fee for a duplicate license\n\nshall be one-half (1/2) the fee of the license.\n\nF. Licensees shall inform by any means acceptable to the\n\nCommissioner of a change of legal name, address or e-mail address\n\nwithin thirty (30) days of the change to permit the Commissioner to\n\ngive proper notice to licensees. A change in legal name or address\n\nsubmitted more than thirty (30) days after the change shall include\n\nan administrative fee of Fifty Dollars ($50.00).
