An applicant for a certificate shall:\n\n1. Submit an application to an Insurance Commissioner; and\n\n2. Pay to the Commissioner an application fee in an amount of\n\nFive Hundred Dollars ($500.00), which shall be sufficient to pay for\n\nthe administrative cost of the certification program and any other\n\ncost associated with carrying out the provisions of the Hospital and\n\nMedical Services Utilization Review Act.\n\nB. The application shall:\n\n1. Be on a form approved by the Commissioner and accompanied by\n\nany supporting documentation that the Commissioner requires; and\n\n2.
