A certificate expires on the first anniversary of its\n\neffective date unless the certificate is renewed for a one-year term\n\nas provided in this section.\n\nB. Before the certificate expires, a certificate may be renewed\n\nfor an additional one-year term, if the applicant:\n\n1. Otherwise is entitled to the certificate;\n\n2. Pays the Insurance Commissioner an annual renewal fee in the\n\namount of Five Hundred Dollars ($500.00);\n\n3. Submits to the Commissioner:\n\na. a renewal application on the form that the\n\nCommissioner requires, and\n\nb. satisfactory evidence of compliance with any\n\nrequirement for certificate renewal;\n\n4. Establishes and maintains a complaint system which has been\n\napproved by the Commissioner and which provides reasonable\n\nprocedures for the resolution of written complaints concerning\n\nutilization review; and\n\n5.
