On or before January 1, 1994, the Insurance Commissioner\n\nshall develop and adopt:\n\n1. Uniform health care claim forms for use by all health care\n\nproviders and carriers in the state; and\n\n2. Uniform standards and procedures for processing such claim\n\nforms in electronic and hard-copy form.\n\nThe Commissioner shall direct all insurers licensed in the state to\n\nbegin using the uniform claim forms by July 1, 1994.\n\nB. The Commissioner shall adopt the health care financing\n\nadministration (HCFA) 1500 form for outpatient billing and claim\n\nsubmission, or its successor, and the uniform billing (UB) 92 form\n\nfor hospital billing and claim submission, or its successor as the\n\nuniform health care claim and billing form for appropriate hospital\n\nand medical expenses, and shall develop uniform forms for other\n\nhealth care provider services, including but not limited to pharmacy\n\nand dental services.\n\nC.
