Provide to its subscribers electronically or in paper copy a\n\nlist of providers upon enrollment and make such list available\n\nelectronically or in paper copy upon reenrollment; provided, a\n\nsubscriber has submitted written assurances that the subscriber can\n\nview and print such electronic copy;\n\n2. Provide to its subscribers notice of any material change in\n\nthe operation of the organization that will affect them directly, no\n\nlater than thirty (30) days after such change;\n\n3. Immediately notify an enrollee in writing of the termination\n\nof the enrollee’s primary care provider and shall provide assistance\n\nto the enrollee in transferring to another participating primary\n\ncare provider; and\n\n4.
