An insurer or prepaid vision plan organization shall not\n\nchange or alter an agreement entered into with a vision care\n\nprovider unless the insurer or organization:\n\n1. Provides notice of any proposed change to the provider\n\nthrough a certified letter or an electronic communication requiring\n\nan electronic signature proving receipt detailing proposed changes\n\nto the vision care provider. A face-to-face or virtual meeting\n\nshall be conducted if requested by the provider. If the changes in\n\nthe contract are not agreed to by the vision care provider within\n\nninety (90) days of the date of the provided notice, the agreement\n\nshall terminate; and\n\n2. Supplies the vision care provider with an explanation of\n\nbenefits and an explanation of payment for services and materials\n\nrendered by the provider upon request, regardless of the provider’s\n\nnetwork status with the vision plan.\n\nB. 1. A new agreement is required to be established and agreed\n\nupon after three or more material changes are made to an existing\n\nagreement from an insurer, vision plan, or vision care discount\n\nplan.\n\n2.
